sañjaya uvāca
evam uktvā tato rājan
mahā-yogeśvaro hariḥ
darśayām āsa pārthāya
paramaṁ rūpam aiśvaram

Word for word: 

sañjayaḥ uvāca — Sañjaya said; evam — thus; uktvā — saying; tataḥ — thereafter; rājan — O King; mahāyogaīśvaraḥ — the most powerful mystic; hariḥ — the Supreme Personality of Godhead, Kṛṣṇa; darśayām āsa — showed; pārthāya — unto Arjuna; paramam — the divine; rūpam aiśvaram — universal form.

Translation: 

Sañjaya said: O King, having spoken thus, the Supreme Lord of all mystic power, the Personality of Godhead, displayed His universal form to Arjuna.

 

evam uktvā, having spoken thus

tato rājan, Here Rajan refers to Dhritarastra

in 11.4 Arjuna used the word Yogeshwar, here Sanjay described by saying mahā-yogeśvaro hariḥ, that Lord Hari who is Maha yogesvar, some yogis can do miracle through mystic power but some of them can do just by trick of hand. 

Darśayām, He reveals what āsa pārthāya, to Partha 

What He reveal? paramaṁ rūpam aiśvaram
